The Universal Care Plan is a dynamic integrated care planning solution for the whole of London. It shares real-time data across all healthcare settings and geographies that make up the 5 ICB’s in the region, and enables shared care plans for a variety of clinical pathways, amongst them end-of-life and sickle cell disease, with others planned in the future.
The technology behind UCP provides interoperability with existing health and care IT systems. Taking an openEHR platform approach with a persistent data layer that is separated from applications, combined with low-code development tools, enables a faster pace of change, local flexibility, and reduces dependencies on application vendors.

Nick Tigere, Head of Programme – Universal Care Plan

With over 20 years of experience in healthcare management, Nick Tigere is currently the Head of Programme for the UCP, which is hosted by NHS South West London ICB. He provides strategic leadership to enable the programme to meet the local, clinical, and regional digital care planning priorities. He holds a Master of Healthcare Management from the University of Surrey, a Bachelor of Science from King’s College London, and a qualification from the HFMA (Healthcare Financial Management Association). He is passionate about improving the quality and accessibility of healthcare for everyone, and is always looking for new ways to leverage technology and data to achieve this.
